Dear Peter,
Thank you so much for asking about my upcoming eyelid surgery. At this point we are still waiting to hear from the insurance which may take about a month. I don't want to do anything until ski season ends anyway so it is OK. It is a little weird to be a patient myself although I do not have any of the fears and trepidation that I see in some of you.
The doctor told me I may have glaucoma so I have been seeing specialists for that. My mother has glaucoma so I am a little worried. So far my workup for that has come up negative so I am thankful. We all take our sight a little for granted. When I start warming up on the threadmill, usually I say a little prayer in my head which includes thanks that everything seems to work and that I have no pain anywhere. I will include thanks for my sight from now on.
One of my patients said hello to my wife and me while waiting in line at chair 7 at Sierra Summit yesterday. I would not have recognized her underneath her helmet and sunglasses. I do enjoy meeting patients outside the office. If you see me and I seem to ignore you, it is for the sake of confidentiality. Some patients are open about surgery/injections, some are not.

Most of the things I do in the office is irreversible. Think about surgery, even something as simple as removing a small growth on the face. Once I cut, there is no going back. that is why we spend so much time making sure we understand what patients want, that they indeed want what they say they want, that patients understand that what we can deliver may not be exactly what they imagined but comes fairly close. A doctor once told me that there is no such thing as a perfect surgery or a perfect surgical result. If you look close enough or question hard enough, you will find something a little less than perfect. A satisfactory result is when the good far outweighs the bad. I am getting off the subject.
Have you ever want to try this or that but are just not sure it would look good or you would like it? Even Botox is not reversible, you just have to wait till the effect goes away. But Juvederm or Restylane fillers are completely reversible. There is a medication called hyaluronidase which when injected into the same area will dissolve Juvederm or Restylane very quickly. You can date Juvederm but pretty much everything else is a commitment.
Case in point, two weeks ago a patient came to me wanting a little upper lip augmentation and some Botox. She came back a few days later and said she liked the Botox but was in a state of panic about her upper lip. I looked at her lip, the shape was good and the volume not at all inappropriate. She was feeling very self conscious, just could not get used to the new upper lip. I reassured her that it looked good even then and that it would probalby settle down in a few days and be just fine. She told me she could not tolerate even one more day looking like she did. Well we injected the hyaluronidase that day. I just saw her two days ago, her upper lip had gone back to the way it was. She told me that she liked the full lip better and will try Juvederm again and keep it this time!

For all you boomers out there, have you noticed people keep asking if you have lost weight even though your weight is the same? That certainly has happened to me more and more. It is the loss of tissue with aging that shows up on the lower 2/3 of the face primarily. Folds and hollow areas start to form. This is especially true if you are fit and are anywhere near ideal body weight.
The answer is fillers, basically volume replacement to counteract the deflation. Think of your face as a nice plump grape in youth, it becomes more like a raisin with volume loss. Facelifting alone is not the answer because you don't get the grape, just a tight raisin.
Lines, folds, lips are handled with traditional fillers such as Radiesse, Restylane, Juvederm. Larger areas such as the cheeks and temples can also be filled with facial implants or fat grafts. The trend now is going away from fat grafts because of unpredictable results towards Sculptra.

Should we continue the monthly specials? I think it is nice to offer something in these economic times but the boss/office manager is concerned that we become known a "discount" practice. What are your thoughts?
