Dear Theresa,  

Well the insurance approved my upper lid surgery. They base this on a visual field examination and close-up photos of the eyes. I am probably going to get it done late April on a Friday and planning to be at work the following Monday. It will be a little swollen and bruised but should not keep me from my usual routine at work. If anyone asks, my standard answer is that I forgot Theresa's birthday, wink, wink...
 
I went to a specialist and the verdict seems to be that I don't have glaucoma after all. Glaucoma is usually not associated with any symptoms in the early stage so for all of you who do not see an eye doctor on a regular basis, please see an optometrist or an ophthalmologist once a year even if your eyes seem fine.

Radiesse vs Juvederm Ultra Plus
 
Juvederm is a hyarulonic acid filler which comes in two flavors. Juvederm Ultra is regular Juvederm and Juvederm Ultra Plus which is a little thicker, lasts a little longer, and costs a little more. Radiesse is a calcium-based filler. They are both FDA-approved for filling the nasolabial folds which run from from the nose to the corner of the mouth.
 
For a typical patient who comes in wishing to have the nasolabial folds filled, I have recommended Radiesse in the past. Juvederm Ultra Plus should also last about 1 year but has the added advantage of reversibility. Which one is better? No one really know because there has been no head to head study comparing the two.
 
I am recruiting a limited number of patients who would be willing to pay for a syringe of Radiesse but will also be getting a syringe of Juvederm Ultra Plus at no cost. We will treat the nasolabial fold on one side with Radiesse and the other with Juvederm Ultra Plus. We will rate the improvement and patient satisfaction at 4 months, 6 months, and 9 months.

What's new?
 
Sculptra finally received FDA approval in July for cosmetic treatment of lines and folds on the face and is now marketed as Sculptra Aesthetic. It is now the longest lasting temporary filler on the market (2-3 years).
 
Juvederm Ultra and Ultra Plus are now available with a pre-mixed anesthetic. Injection still require a topical numbing cream.
 
A division of Johson & Johnson, Ortho Dermatologics, sent out notice that Evolence--a wrinkle filler--was being taken off the market and the product is up for sale. Evolence has failed to gain traction in a market crowded with alternatives like Restylane and Juvederm. On RealSelf.com patients have complained about Evolence results more than any comparable wrinkle treatment. The difficulty of injecting Evolence may have led to its demise in the aesthetics market.  Dermatologist Dr. Mary Lupo explains that "Evolence is a very stiff product...It also must be injected with a larger needle and requires a lot of massage to prevent lumping, so bruising is very likely. Lumping is a real possibility."

Thicker is better for deeper folds 
 
Juvederm comes in two flavors: Ultra and Ultra Plus. The Ultra Plus is a little more expensive but is thicker, lasts longer, and is meant for deeper folds.
 
The patient in the photo had the same volume of  Ultra Plus on the right nasolabial fold and Ultra on the left. You can clearly see the left side looks deeper than the right. Both sides feel the same. Both Ultra and Ultra Plus are reversible for added security.
 
Should we continue the monthly specials? The feedback have been overwhelmingly in favor of continuing our specials. So we will still have them. Thank you all, we love you.
