To tell or not to tell?
 
Recent survey shows changing societal attitude
 
A survey of 1200 people by the American Academy of Cosmetic Surgery shows that 71% believe society is less judgmental about cosmetic surgery than it was 5 years ago. The survey also says that about 62% feel that society's attitude toward cosmetic surgery makes them feel more comfortable about getting a procedure.
 
In the AACS survey, participants were asked what influences their decision to talk about cosmetic surgery. More than 41% said they prefer to be open/honest about their new look while 29% said they are proud of their new look and want to talk about it. Less than 17% said they had a fear of being judged by others.
 
Among invasive procedures, the top three most accepted surgeries are breast augmentation, face lift and abdominoplasty. Among non-invasive procedures, Botox and laser hair removal both rank very high in acceptance (over 60%). Additionally, more than 50% would tell family, while almost 40% would tell anyone and about 30% would tell friends.

Beware of unlicensed practitioners

Solange Magnano, a former Miss Argentina, died on November 29, 2009 from complications of buttock augmentation. In her late 30s and the mother of young twins, she went to a clinic in Buenos Aires, Argentina. She died of a pulmonary embolism after receiving injections of a "liquid" named in some news report as liquid silicone and in others as polymethylmethacrylate, or PMMA.
Cases like Ms Magnano's are less likely to occur in the US, where the FDA has not approved the use of any fillers for buttock augmentation. However things can go wrong here too, especially when unlicensed, non-medical providers use unapproved products, such as industrial silicone to augment the buttocks. Closer to home, even celebrities such as Priscilla Presley have been irreparably harmed by the use of silicone in the face.

Learn from other people's experience 
 
Realself.com is a site where patients can ask questions and have them answered by physicians, read/post reviews on surgical procedures, lasers, fillers, Botox, etc... I participate as a physician by answering questions, my answers appear on the site as well as on my own website (see this link).
As with all Internet reviews by users, happy patients are less likely to post. Still I find it useful to look at satisfaction rate on this site to evaluate new technologies or procedures. Anything with a satisfaction rate around 50% or above is probably quite effective. For instance, on this site Radiesse has 48% satisfaction, Juvederm is 56%, Botox is 64%, eyelid surgery is 57%. My own patient satisfaction rate for these procedures are in the high 90's. Take Velashape/Velasmooth, a procedure that promises to improve the appearance of cellulite, only has 29% satisfaction rate. That tells me the procedure is not ready for prime time.
