Dear Peter,
Many patients have come up to me to say that my wife really looks good following upper lid surgery. Many thanks for that. She is a little embarrassed by the attention. Fresno being the biggest little town in America, gone are the days when she can walk around incognito. She is fond of reminding me that she married me when I was young and struggling.
In this issue, you will hear some stories from our patients. They are fun little vignettes that celebrate what we at Faces! are about: celebration of life, looking good and feeling good. If you have a good story of your own, please send it to me. I will pick out the best ones and publish them in this forum in the future (your privacy will be respected).

J. C., age 50, owns a successful business in town. He has had some filler and Botox injections, but no surgery. He told me this story: "The other day I was at a gas station paying for the gas with my credit card. The attendant asked for an ID so I showed him my driver's license. He looked at the photo, then looked at me, then looked at the photo again. He said: "This isn't you. This man in the photo is an old man. You are a young guy. You stole this, huh?" I told him that it's me, showed him all my other ID's but he did not believe me. Finally I had to tell him to call the cops then he believed me. He told me I should get my driver's license redone with a new photo."
S. P., age 65, had a lower lid blepharoplasty with fat repositioning, lower facelift and necklift. She gave me a hug when she saw the before and after photos. This is what she said: "The other day I was at the supermarket with my daughter who is 40 years old. The cashier asked if we were sisters."
N. M., age 57, had her upper lids done and some Botox injections. She came in yesterday with a Cheshire cat grin and said: "I want to show you something." She pulled out her old and new driver's license and laid them side-by-side. A picture, even a driver's license picture, is worth a thousand words. There is the old N. M. and the new N. M., more youthful, stylish, confident, staring out at us. She was practically dancing around in the office.

Updates on Dysport

A worthy competitor for Botox
The market for Botox worldwide is over 1 billion dollars. This kind of money eventually brings about competition. Dysport, as you know, has just been approved by the FDA for cosmetic treatment of brow wrinkles. Two others are in the pipeline: Purtox and Xeomin. I just went to a conference to learn more about Dysport from Dr Corey Maas, one of the original investigators for the Botox study. He is now an investigator for the Dysport study for the FDA.
He thinks that Dysport is a good product. It is at least as good as Botox. Since there are no good head to head comparison studies between the two, we cannot say that one is better than the other. What he has noticed is that Dysport tends to work faster than Botox (in 1-2 days instead of 2-3 days) and tends to diffuse a bit more. What this means is that the effect spreads a little more from the injection site. As an injector, we can use this to our advantage. There seems to be no difference in the rate of complications, all of which are temporary. The median duration of action for Dysport is 4 months (the point at which it stops working for 50% of patients). In Dr Maas' practice, he does not push one over the other and presents Dysport as an alternative to Botox.
I have used Botox forever and am very comfortable with it. I will continue to use mainly Botox but will offer Dysport as an alternative. There are patients who always want to try the latest product and we want to accommodate them as well. Our pricing for Dysport is 10.00 for 2.5u (equivalent to 1 unit of Botox which is normally 11.00/unit).

| What do we offer for younger patients? |
I was talking to one of my patients the other day. She is an older actress who is just now breaking into the business. She mentions that she would like to refer patients to me but she hangs out with mostly with younger men and women who don't seem to need anything. We do offer permanent laser hair reduction for men who prefer a clean torso free of hair or have issues with ingrown hair on the face. We also work on young women for bikini lines and armpits. We also work on acne who do not respond to conservative treatments by using medical grade skin care or light treaments that will shrink the oil glands and kill the skin bacteria responsible for the inflammation.
Often what distinguishes an absolute hunk like George Clooney or a timeless beauty like Angelina Jolie from us average Joe is bone structure, i.e. the right volume in the right place. We offer implants that will strenghthen the jawline or add drama to a so-so cheekbone. In the pictures below, on the left we have the real photo of Mr Clooney. On the right is what he may have looked like if his chin were naturally a bit less manly and chiseled. Implants can correct all that and more.
Lower eyelid bags can be present at very young age. I had them and hated them even in my teens. I had them removed in my early thirties. Has anyone looked at Oprah lately? Someone please tell her she would look so much better if she saw Dr Truong who would fix her lower lids so they can stop airbrushing her photos. We will guarantee complete discretion!
Granted they can be overdone but rare is the woman who would not benefit from slight or moderate lip augmentation to increase fullness or enhance the shape. I can go on and on.
